چکیده

The purpose of this study was to determine the accuracy of alcohol swabs in assessing dermatome levels in comparison to pinprick after subarachnoid block. Room temperature alcohol swabs and pinprick were used to determine dermatome levels at 5, 10, and 15 minutes after a 15-mg hyperbaric bupivacaine subarachnoid block was administered with the patient in the sitting position. The sample population consisted of 53 men scheduled for elective transurethral resection of the prostate or bladder tumor with an ASA classification of I to IV. Subjects were assessed while they were in the preoperative holding area for the ability to discriminate pinprick and cold sensation. Data were analyzed using the Wilcoxon signed rank test. There was a statistically significant difference between alcohol and pinprick at the 10- and 15-minute assessments. (P < .05).

Effective dermatomal blockade after subcostal transversus abdominis plane block.

INTRODUCTION The ultrasound-guided transversus abdominis plane (TAP) block is used to treat postoperative pain after abdominal surgery. Abdominal wall sensory nerves are anaesthetised by injecting local anaesthetics into the neurofascial plane between the internal oblique and the transversus abdominis muscles. Sensory assessment of a TAP block may guide the decision on the extent of the block. ...

Quantitative assessment of differential sensory blockade after lumbar epidural lidocaine.

A cutaneous current perception threshold (CPT) sensory testing device measures both large and small diameter sensory nerve fiber function and may be useful in evaluating differential neural blockade. Eight subjects received both lumbar epidural saline and lumbar epidural lidocaine. Five milliliters of normal saline was administered and the CPTs were measured. After the saline, 10 mL of 2% plain...

Dexamethasone added to lidocaine prolongs axillary brachial plexus blockade.

Different additives have been used to prolong regional blockade. We designed a prospective, randomized, double-blind study to evaluate the effect of dexamethasone added to lidocaine on the onset and duration of axillary brachial plexus block. Sixty patients scheduled for elective hand and forearm surgery under axillary brachial plexus block were randomly allocated to receive either 34 mL lidoca...

Time sequence of sensory changes after upper extremity block: swelling sensation is an early and accurate predictor of success.

BACKGROUND Sensory assessment to estimate spread and effectiveness of a peripheral nerve block is difficult because no clinical test is specific for small sensory fibers. Occurrence of a swelling illusion (SI) during a peripheral nerve block corresponds to the impairment of small sensory fibers. The authors investigated the usefulness of SI in predicting successful peripheral nerve block by ass...
